- Medbringe på ansøgningsdagen:
o Barnet.
o Fødselsattest.
o Hvis ikke begge forældrer møder op ved ansøgning, underskrevet kopi af den fraværende forælders pas.
o Ansøgningsskema hvor begge forældrer har underskrevet.
o Dokumentation som godtgør familietilhørsforholdet, f.eks. japansk juuminhyou og vielsesattest (eller koseki i stedet for vielsesattest, hvis den ene forælder er japaner). Hvis ægteskabet er registreret i Danmark kan ambassaden verificere det i cpr-registret. I så fald behøves ikke vielsesattest.
