• brugeren hjælpes til at genvinde tabte
kompetencer og opnå størst mulig grad
af selvstændighed og livskvalitet, både
socialt, psykisk og fysisk.
• hjælpen ydes i et samarbejde mellem
borger, pårørende og fagfolk.
• hjælpen ydes med udgangspunkt i borgerens
samlede livssituation og bestå af
en koordineret, sammenhængende og
vidensbaseret indsats.
